P.S.I. (Post-Secondary Institution) Form
All Grads should complete the Ministry of Education PSI form to give the Ministry permission to send your transcript of marks to your post-secondary institution(s) of choice. (Maximum 6 institutions.) You should do this prior to March 1, 2014.
Steps for completing the form:
You will need your PEN (personal education number), found on your report card or available from Mrs. Wielemaker.
The PSI Selections Form is found on the Student Secure Web (SSW) at
Students who do not yet have a SSW account will need to create one on the above website prior to gaining access to the PSI form. If you have already created an account in order to look at your provincial exam marks, you must use the same password you used in the past. If you have forgotten your password, you will have to create a new password, which might take some patience and persistence on your part.
Click on the menu option "Post Secondary institutions Selections".
Most of you will check the box that refers to BC PSI's.
Print a copy of the confirmation page at the end of the process and keep it in a safe place. In past, universities have claimed they did not receive marks; it's important that you have proof that you instructed the Ministry to send the marks.
You may go back to the SSW at any time to add more institutions, up till July 15, 2014. You can also view your selections at any time - use the "View PSI Selections" menu option.
